Iliotibial Band Friction Syndrome

Introduction
  • Caused by rubbing between the iliotibial band and the lateral femoral condyle
  • Often seen in runners and cyclist
  • Also known as "runner's knee"
Presentation
  • Physical exam
    • localized tenderness over lateral knee that is worse with the knee flexed to 30°
    • Ober test 
      • patient on side, symptomatic side up, hip flexed and abducted, knee flexed
      • pain, tightness, or clicking when hip is brought into extension and adduction
Treatment
  • Nonoperative
    • physical therapy & steroid injections
      • rehabilitation is often successful.
  • Operative
    • elipitical surgical excision of  iliotibial band 
    • Z plasty of iliotibial band        
      • only indicated in refractoy cases
