Hip Labral Tear

Introduction
  • Highest incidence in patients with acetabular dysplasia
    • leads to intra-articular snapping hip 
  • Differential
    • synovitis
    • iliopsoas tendonitis
Presentation
  • Symptoms
    • mechanical hip pain and snapping
    • may have vague hip pain
    • symptoms relieved with intra-articular injections
  • Physical exam
    • pain and clicking with hip flexion, adduction and internal rotation
Imaging
  • MRI 
    • more sensitive when combined with arthrogram 
Treatment
  • Diagnosis
    • symptoms relieved with intra-articular injections
    • arthroscopy is the gold standard for diagnosis
  • Nonoperative
    • rest, NSAIDS, physical therapy, steroid injections
      • indicated as a first line of treatment
  • Operative
    • arthroscopic repair
      • indications
        • severre symptoms that have failed to improve with nonoperative modalities
      • technique
        • can be debrided or repaired depending on extent of tear
        • underlaying hip pathology (e.g. FAI) should also be addressed at time of surgery
