This video demonstrates the repair of a large anterior bankart tear which extends under the biceps tendon insertion. The biceps involvement represents a significant SLAP tear. Surgery begins with elevation of the torn labrum. It has become stuck down in the incorrect position and in order to repair the labrum to its normal position it must be released until it can be easily repositioned. Next a burr is used to abraid the glenoid margin so that the cartilage can heal back to the bone once it is placed back in its correct position. Anchors are introduced through a cannula; a drill creates a hole within the bone and the anchor is then tapped into position. A passing suture is placed through the capsule and around the torn labrum. This is then used to pass the suture attached to the anchor through the tissue to be repaired. This is also done for the biceps anchor. The completed repair can be seen with the three anchors for the bankart tear and two for the biceps tear in position. The labrum can be seen to be back in its correct position and blocking further anterior dislocation of the humeral head.
