Milestone Skill Evaluations (MSEs) - Putting Residents in the Surgical Driver's Seat

1. Residents need to demonstrate ~ 120 skills per year, or 2-3 skill per week.

2. Milestone Skill Evaluations represent a very small % of OR teaching - 99% of the time will not change.

3. Milestone Skill evaluations should be an opportunity to take a time out from the normal hustle of residency and focus on high-quality learning/teaching for a small part of the procedure in which the resident is well prepared.


RULES OF THE ROAD

1. Residents should not be evaluated on more than one skill per procedure. (this is about quality, not quantity)

2. Residents should not complete more than 2 skills per day.

3. Residents should requst the skill evaluation to the faculty the day before the case. The resident should remind the faculty at the beginning of the case which skill he wants to be evaluated on.

4. Faculty should grade the evaluation by the end of the day. It should take less than 15 seconds on their mobile phone.


POOR GRADES ARE AN OPPORTUNITY TO LEARN

If a resident does not do the procedure competently and independently than he should receive a poor grade.

Poor grades should be viewed as a good thing - an opportunity to be better.

If given a poor grade, the resident repeats the procedure the next time. Once given a passing grade, the poor grade is deleted from the record permanently. NOBODY can ever see it again.
