The radiograph shows a calcaneonavicular coalition. Surgical treatment of the coalition should result in symptom improvement. Nonsurgical treatment has been tried and failed. She has pain with daily activities, so discontinuing gymnastics is not likely to help. Cast treatment may provide temporary relief but does not provide relief once the cast is removed. Continued physical therapy is not likely to help because it has previously failed.
