The technique for the trochanteric osteotomy during surgical dislocation procedures should be performed with a maximal thickness of 1.5 cm. The interval between the gluteus medius and piriformis is not developed prior to the osteotomy. At the most proximal extent of the osteotomy, the saw should exit anterior to the posterior insertion of the gluteus medius to ensure the osteotomy does not inadvertently penetrate into the short external rotators, thus preserving the deep branch of the medial femoral circumflex artery.
