Current literature supports the use of reinforcement cages for the reconstruction of failed, loosened acetabular components associated with major bone loss as seen in this patient. Although results of revision using the so-called jumbo cup with screws generally have been good, the amount of bone loss and medial wall penetration shown here and the likelihood of pelvic discontinuity precludes the use of that technique. With either technique, bone grafting of remaining defects is recommended.
