Open chain refers to exercises where the terminal appendage is unsupported. For knee surgery, this means exercises where the foot is free. If the foot is supported on the ground or a pedal (bicycle) this would be considered "closed chain".

The reference by Wright et al is a systematic review of the literature which addresses rehabilitation programs. It shows that closed chain programs are better for early rehabilitation after ACL reconstruction.
