A 62-year-old woman is on follow-up with your office for recurrent plantar ulceration for 2 years that has been refractory to offloading and total-contact casting. She has a history of insulin-dependent diabetes mellitus. Examination reveals a 1.5cm ulcer with a pink base and undermined edges with no palpable bone. She is unable to detect a Semmes-Weinstein 5.07 monofilament on multiple sites on the plantar foot. A recent clinical photograph and radiograph are shown in Figures A and B.
