A 53-year-old man has bilateral hallux valgus that has failed non-operative treatment. He undergoes bilateral staged hallux valgus corrections. Following a proximal osteotomy, his right foot has a squared lateral metatarsal head and the tibial sesamoid is noted to migrate under the metatarsal head. On the left foot, he has a rounded metatarsal head and the tibial sesamoid overlaps the lateral border of the metatarsal head. At 1 year, recurrent painful hallux valgus is noted on the right side.
