A 33-year-old male reports a persistent 2-month history of forefoot pain and swelling after he stepped on a nail while jogging and wearing rubber-soled athletic shoes. He went to the emergency department and was given tetanus prophylaxis and the appropriate prophylactic antibiotics. Examination reveals a healed puncture wound under the plantar forefoot measuring 2 mm in diameter that is moderately tender. There is no erythema or fluctuance. Lab values are within normal limits.
