The clinical photo and radiograph are consistent with a Grade III open tibia fracture. The referenced study by the LEAP group reviews 527 patients with severe lower extremity fractures and found that the most important factor in determining the ability to salvage the extremity remains the severity of the soft tissue injury of that extremity. Bone loss has been shown to have no effect on the eventual outcome (amputation versus salvage). Similarly, plantar sensation at presentation has no bearing on final outcome, and in the LEAP study, often either partially or fully returned.
