A 54-year-old female presents to the clinic for continued management of low back pain and left leg weakness. The patient has previously attempted physical therapy and NSAIDs with no improvement. She initially received spinal injections into the left L4-5 foramen with significant relief of pain, however, her most recent injection provided little benefit. On examination, there is 4/5 strength with left ankle dorsiflexion compared to the contralateral side. Radiographs and MRIs are obtained (Figures 1-3). The patient subsequently undergoes surgery (Figures 4 & 5). At her two-week postoperative visit, she reported burning pain and occasional paresthesias to the groin and left inner thigh.
