A 52-year-old male construction worker presents to your spine clinic with complaints of new-onset low back pain with radiation into the anteromedial distal thigh, adjacent to the knee. He states it began when he bent over to pick up a heavy toolbox. He denies having prior treatment for his lumbar spine. On exam, he has a positive straight leg raise and tenderness to palpation over his lumbar paraspinal muscles. He has 2+ patellar reflexes on the right and left.
