Aseptic loosening occurs through four main steps: (1) particulate debris formation; (2) macrophage activated osteoclastogenesis and osteolysis; (3) prosthesis micromotion; (4) particulate debris dissemination. The most commonly particulate generated through adhesive wear following a TKA is UHMWPE. Macrophage phagocytize the wear particles and debris and subsequently release several cytokines including TNF-a.
