QID 219112 (Type "219112" in App Search)
A 65-year-old male presents to your office with complaints of persistent foot pain and swelling which has been present now for the last 2 months. He does report a long history of type II diabetes but does not know his most recent hemoglobin A1C. The sensation in his feet is diminished with Semmes-Weinstein monofilament testing bilaterally. He notes that when he wakes up in the morning the swelling and erythema are much better than before he goes to bed. His radiographs are seen in Figure A. No fluctuance is noted clinically and there are no fluid collections seen on the non contrast CT ordered by the ED. He has not had any prior treatment for this issue. What is the next best step in the management of this patient?
