A 30-year-old male presents to your clinic with complaints of left knee pain that has been present for years. He states he underwent a failed meniscal repair and subsequent subtotal meniscectomy when he was 20 years old due to a collegiate football injury. An AP radiograph is seen in Figure A. No patellofemoral arthritis is noted on the lateral radiograph. On exam, his pain is focal over the medial joint line. He has a 5° flexion contracture and varus alignment of the lower extremity when standing. He has been taking acetaminophen, naproxen, and has had a corticosteroid injection without much improvement. He is hoping to get back to playing recreational soccer and flag football. What is the next best treatment option?
