The patient works as a mechanic and is having difficulty with work secondary to functional deficits of the hand. On exam, there is clawing present. The patient is unable to actively extend the affected digits at the proximal interphalangeal joint (PIPJ). While asking the patient to extend the digits, you perform the maneuver demonstrated in Figure A and the patient is still unable to actively extend at the PIPJs.
