A 55-year-old female presents with low back pain. The pain has been present for 3 years, and is located in the lower back with radiation down both legs. The pain is intermittent and worsens with prolonged walking or activity. She is no longer able to work as a realtor as she cannot walk for more than 10 minutes before being limited by leg pain. Nonoperative treatment with physical therapy, anti-inflammatory, and muscle relaxant medications have not provided significant relief. Her radiographs and a transverse cut of L4/5 MRI are shown in Figures A and B. What treatment should you offer her to have the best long-term improvement in bodily pain and physical function?
