A 28-year-old male IV drug user presents to the ED with a forearm abscess after injecting heroin. Incision and drainage is performed and cultures reveal Methicillin-sensitive Staphylococcus Aureus (MSSA). He is treated with intravenous Cefazolin and transitioned to oral Cephalexin as an outpatient. He returns to the ED 2 months later with a similar forearm abscess. Repeat incision and drainage is performed and cultures/sensitivities are again obtained. Staphylococcus aureus is identified, however, it is now resistant to Methicillin and several other penicillins and cephalosporins.
