A 40-year-old recreational bicyclist presents to the emergency department with severe lower back and bilateral gluteal pain as well as difficulty ambulating after flipping over his handlebars when a motor vehicle cut him off. Physical examination is significant for midline tenderness at the thoracolumbar junction, no palpable step-offs, marked paraspinal muscle spasm, and otherwise intact neurologic function and rectal tone. A CT is obtained and is shown in Figures A and B. An MRI confirms intact posterior elements.
