A healthy 66-year-old female with no significant medical co-morbidities presents to your clinic with long-standing right ankle pain. She enjoys hiking and tries to stay active. She has tried NSAIDs, physical therapy and bracing, none of which provide much relief. Intra-articular injection of corticosteroid and lidocaine did alleviate some of her symptoms albeit temporarily. On exam, she has dorsiflexion to 5°, plantarflexion to 15°, inversion to 10°, and eversion to 10°. She has no pain with subtalar motion but significant pain with tibiotalar motion. Figures A-C are her most recent ankle Xrays. What is the optimal surgical option to maximize functional outcomes and return to comfortable gait on uneven surfaces for this patient?
