A young patient with a peripheral bucket-handle meniscal tear should be treated with meniscal repair. While there is a trend towards using more all-inside devices for smaller tears, the standard for bucket-handle tears is an inside-out repair. Vertical mattress sutures have been found to be the strongest suture configuration. According to Boenisch et al. compared bioabsorbable arrows to vertical and horizontal sutures. Sutures were stronger and the vertical configuration was the strongest in terms of pull-out. Rankin et al. studied various methods are available for repair of meniscal tears: a biodegradable meniscal implant without sutures (Biofix meniscus arrow), a suture anchor device (T-fix), and horizontal and vertical mattress sutures. They found that stures were stronger than devices and that vertical mattress sutures are biomechanically stronger than horizontal sutures.
