I would consider it a SH IV as it crosses the epiphysis and metaphysis. I would put a cannulated screw (or two) across the epiphysis and one in the metaphysis.

Nice result! Would’ve used partially threaded screws though. Medial as well in the epiphysis just because it’s easier to place the wire.

Thanks to sharing. Triplane fracture .My strategy is ORIF to Remove entrapped periosteum 1-anterolateral approach to reduce and stabilize the epiphyseal fracture.2 Temporarily stabilization of the fracture using K-wire 3- fist screw (epiphyseal screw )is placed from the lateral side and runs paralle...l to the physis : perpendicular to the fracture line. 4- metaphyseal fracture is fixed by an anterolateral to posterior screw .
