"We report the case of a 28-year-old female with signs and symptoms consistent with a SBRN com- pression, who underwent operative decompression. Intra-operatively it was noted that the main part of the SBRN travelled in its usual place deep to the BR before emerging from under the tendon 6 cm above the radial styloid. At this level there was a condensa- tion of the deep fascia passing in a transverse direc- tion between the ECRL and BR. Another, smaller, nerve was noted to emerge 3 cm proximal to this point from between two leaves of the tendon of the BR. This smaller nerve joined the SBRN where it emerged between the BR and ECRL and ran distally with it. Proximal dissection of this smaller nerve revealed that it passed through the BR tendon and pursued an entirely intra-muscular course (Figures 1 & 2). Its source was not determined as it lay proximal to the surgical field."
