A 43-year-old male undergoes a single-incision distal biceps repair for a tear that occurred only one week earlier. He did not receive a peripheral nerve block prior to surgery. During the surgery, the tendon was easily identified and tagged with sutures in order to mobilize it to the radial tuberosity. The forearm was hyper-supinated in order to gain exposure and visualization of the tuberosity for reinsertion. Hohmann retractors were placed on each side of the radius for visualization. The biceps tendon was reattached using suture anchors into the tuberosity. Postoperatively, the patient is unable to give a “thumbs up”. What is the most likely reason for the patient’s inability to extend their thumb?
