The clinical presentation and images are consistent with a talocalcaneal coalition. Tarsal coalition, most commonly occurring as a talocalcaneal or calcaneovnavicular coalition, is the leading cause of peroneal spastic flatfoot and leads to a rigid pes planus deformity of the foot.

Harris discusses cases of a talocalcaneal bridge causing a rigid valgus foot deformity and the difficulties of diagnosing this condition both clinically and radiographically.

Crim discusses the common radiographic findings associated with different tarsal coalitions. In particular, she mentions the "C-sign", which is shown in Illustration A. This is a radiodense outline between the talus and calcaneus best seen on the lateral foot radiograph that forms the shape of a "C". This is indicative of a talocalcaneal coalition.
