A 62-year-old male presents with left hip and groin pain that was recalcitrant to conservative measures. He undergoes uncomplicated left total hip replacement and does well post-operatively. He returns to the clinic ten years later and would like to proceed with a right hip replacement. His updated radiographs are shown in Figure A. The patient notes he had a posterior spinal fusion procedure (L3-5) two years ago without any complications. He has a normal overall spine alignment but a stiff lumbar spine, as denoted from the lack of change in his sacral slope when compared between standing and seated positions (Figure B). Compared to his left total hip arthroplasty, how can the positioning of the final implants used for right total hip arthroplasty be adjusted to minimize complications associated with this patient’s stiff spine?
