HPI
The patient is a 74-year-old female who underwent a primary total hip arthroplasty in 2013. She had an uneventful postoperative course and remained fully functional for many years. Over the past three months, she has developed progressive and severe right hip pain, accompanied by a marked decline in mobility. She now reports inability to walk without assistance. There was no recent trauma.

PE
On examination, the previous lateral approach incision is well-healed, with no erythema, swelling, or signs of local infection. There is significant pain with passive and active hip motion. No limb deformity is noted. Neurovascular status is intact, with normal distal sensation, motor function, and palpable peripheral pulses.
