Type 3 left supracondylar fracture

HPI

The child was playing and climbing on the fence. He fell down on an outstretched hand,and started having a left elbow swelling,pain and inability to flex and extend the elbow.

PMH

The past medical history is unremarkable.

PE

The patient was awake,alert and not in distress. Had an above elbow backslab,which upon removal showed a swelling around the elbow,a deformity around the elbow and no visible ecchymosis. Range of motion could not be assessed due to pain.

The patient was taken to theatre on the second day of admission. Under general anaesthesia an closed reduction was done by traction- counter traction for about 5 minutes and then immobilising it by flexing the elbow, supinating the forearm;and lastly tying the wrist to the proximal arm with a crepe bandage. Using 1.6 mm k wires two lateral parallel k wires were passed, a medial k wire was passed too. The entry points were cleaned with alcohol and then paraffin gauzes were wrapped around the k wires. An above elbow backslab was applied.
