The medial approach to the arm is ideal for a humeral rotational osteotomy for patients with brachial plexus birth injuries (BPBI). It allows for unrestricted rotation with a cosmetic scar and a flat area of bone for placing the plate without encumbrance from a traversing radial nerve. The interval is between the ulnar and median nerves and between the biceps and triceps along the medial inter muscular septum.
