Cerebral palsy with crouched gait

HPI

9F with spastic diplegic cerebral palsy, GMFCS III, with severe crouched gait. Primarily mobilizes by crawling when at home. Otherwise, uses assistive devices. She has undergone bilateral hamstring Botox injections with casting in addition to bilateral hamstring and iliopsoas lengthenings with no improvement in her symptoms. Currently using KAFOs.

PMH

Patent ductus arterious s/p ligation, precocious puberty

PE

Bilateral knees with a popliteal angle of 90°. Enlarged prepatellar bursa bilaterally. Flexion contracture of 60° bilaterally. Bilateral hip flexion contractures of 15°.
