Acute Groin Pain 3 years s/p THA in 75M

HPI

A 75-year-old male, with a history of a Right THA done in July of 2021, presents with acute right groin pain. He had been doing well since surgery, until 3 days before the visit he was going to sit down on the toilet and felt a pop in his right hip. He states the toilet was much lower than he was used to. Since this event, he states his pain is excruciating and he cannot bear any weight. During his original THA in 2021, he was found to have a massive abductor tear of the anterior 2/3rd of the abductor's tendon which was repaired with suture anchors. Regarding a workup for infection, ESR and CRP are normal.

PMH

His past medical history is only significant for aortic stenosis.

PE

On physical exam, the posterolateral wound is well healed with no evidence of infection. There is fluctuance and swelling over the anterior thigh going just above the suprapatellar pouch. There is no skin discoloration. He has a painful arc of motion throughout. Distally he is NVI.
