low back pain radiating to b/l lower limb (L>R)since 9 months Associated numbness in b/l lower limb below L2. Pain aggravated by bending forward, with activities. VAS 7/10 normal bowel/bladder habits

SLRT b/l 60 degrees Left EHL 4/5 sensory deficit by 10 % below L2 b/l b/l knee and ankle jerk absent stepping in L2 L3 jxn
