HPI

Patient is a 43 year old male with 10 year history of left hip pain, groin pain, and restriction in motion. He had hip scope 3 yrs ago which led to no significant improvement. He is married and still plays recreational hockey.

PMH

Past medical history / review of systems negative

PE

Decreased ROM relative to right hip.
