Ankle Fracture in 40M

HPI

40 yo athletic male who jumped from a boulder into shallow water presents with swelling and pain in his right ankle. He has a history of a severe ankle sprain to the same ankle 20 years ago however after healing that injury he had no issues whatsoever with regards to pain or instability. Following the injury he hiked 5 miles through difficult conditions including crossing a river 7 times in waist deep water wearing only an aluminum stirrup splint and taping. Since the injury and over the past two weeks he has been walking in a CAM walking boot, takes noting for pain and only has a mild discomfort over the distal fibula. He has also started back to doing heavy squats wearing only high top shoes. He occasionally feels a mild painless click in his ankle when he bears weight.

PMH

Healthy without any medical problems.

PE

Tenderness over the distal fibula, neuro intact, medial and lateral swelling with dependent bruising, no medial sided tenderness.
